How to Fill EPF Form 10D?
Form 10D can only be filled online, and the respective member has to fill in the following details in the form with relevant information:
- The First Question Is “By Whom the Pension Is Claimed”
In this field, one has to fill in the following details:- If you are a Member of EPF then write your name.
- If you are a Widow/Widower of the member then write it.
- If you are an Orphan or Major
- If you are a Guardian of the member of EPF
- If you are a Nominee, nominated by the member.
- Write a Dependent Parent, if you are the one claiming the pension.
- The Second Question Is, “Type of Pension Claimed”
There are seven different types of pensions, and you have to name the type of pension you are filing the form 10D for: Here are the types of pension one can accumulate from Form 10D\

- Reduced Pension – Reduced pension is a monthly pension at a discounted rate of 4% p.a. This can only be claimed after the age of 50 years.
- Superannuation Pension – It can also be claimed as a monthly pension but only after retirement at the age of 58 years.
- Widow/Widower and Children’s Pension – This pension can be claimed on a monthly basis after the death of a member. This pension helps widows in running their houses and managing the expenses of children.
- Orphan Pension – It is claimed on a monthly basis with a benefit for the surviving sons or daughters after the member or employee is deceased. The age of the employee must be up to 25 years of age.
- Dependent Parents – Parents who are or were dependent on their child’s earning can claim for this type of pension after their child is deceased and have left no family behind.
- Disablement Pension – Disablement pension is achievable as an early monthly pension amount if the employee is permanently ill or has complete bodily disablement.
- Nominee Pension – A person nominated by the member as the heir of his EPF amount if he or she dies accidentally or due to other issues. Nominee pension also worked as a monthly pension especially, for those who do not have any family members.

- The Third Question Is About Filling, “Member Details”
You have to fill in the following details:
- Name of the member
- Gender of the member
- Marital status of the member
- Date of birth and exact age of the member
- Father’s or husband’s name of the member (fill accordingly)
- In The Fourth Section You Have to Fill, The “EPF Account Details”
- Recognized Organization Code (RO Code)
- Office Code
- Establishment Code Number
- Account nUmber of the member
- In The Fifth Section, You Have to Enter The “Name & Address Of The Establishment”
Just enter the name of the company and the address where the member has worked prior to joining. - In The Sixth Section, You Have to Insert “Date Of Leaving The Service”
Make sure to enter the last working day at the previous company or organization in the format – DD/MM/YYYY - In The Seventh Segment, Describe The “Reason for Leaving The Service”
One who is claiming the pension has to enter the reason why the member left their previous job. Is it due to permanent physical damage, entire body disability, and so on, only then the member is entitled to the disablement pension. if there is another reason for leaving the job then mention it as well. - In The Eighth Section, One Has to Enter The “Address”
You need to mention the address, the pension claimer of the member is living currently for communication purposes. - In The First Part of The Eighth Section, You Have to Write About “Reduced Pension”
You have to mention if a member has worked for less than 58 years of age or has not completed the age of 58 years as per the date of birth mentioned on the form then while claiming the pension they have to write the commencement date of the pension. - In The Ninth Section, You Have to Select The “Commutation Option Of 1/3 Pension”
A commutation is an option for the member to receive the entire sum in one go or keep receiving the amount on a monthly basis for the rest of your life. Another option is, a member can get 30% of his pension at one go and use the rest of it on a monthly basis. - In The Tenth Section Select The ” Option For Return Of Capital”
You have to either tick the yes or no option depending upon whether you want your complete pension at once or not.
- In The Eleventh Section, ” Mention Nominee for Return Of Capital”
Write the name of the nominee who will collect the pension in case the member is dead or deceased. In this, you have to mention the name, relation, date of birth and address of the nominee. - In The Twelfth Section, “Write Family Particulars”
You have to write the details like name, relation, date of birth, and address of the family member. In case of the death of an EPF member, his or her family members will be eligible to receive the pension. - In The Thirteenth Section, The Claimer Has To “Enter the Date Of The Deceased Member Of The EPF (If Applicable)
This field will only be filled when the EPF member is no more. And, in support of him or her being deceased, the claim has to attest to the death certificate of the member. - In The Fourteenth Section, One Has to Write “Details Of The Account Opened
You have to write the details related to bank accounts such as – name of the bak, name of the branch of the bank, and complete postal address with a PIN of that area (where the bank branch is situated)
The first section of the fourteenth part has to be filled only if the claimer is a nominee of the deceased EPF member. - In The Fifteenth Section, One Has to Mention “Detail Of Scheme Certificate
Claimer has to write down the details of the Scheme Certificate (member already has the possession). In detail, you have to mention the Scheme Certificate Control Number and the authority that issued the Scheme Certificate. - In The Sixteenth Segment, You Have To Give Details About Whether The “Pension Is Being Drawn Under E.P.S, 1995”

- The applicant or the claimer has to verify all the details by signing the form at the end and also make it signed by the employer.
- Filing Form 10D is a long process, therefore, there is no online form filing facility available.
- In section 2 there is a table that is required to be filled by the employer. The employer will submit the form while enclosing it with all the necessary documents required for processing Form 10D in the EPFO regional office.

What Are The Criteria For Applying Online For Form 10D?
To get your monthly pension under eps-95, one has to submit Form 10D. You can submit this form both offline and online using the Unified Member Portal. But still, there are some essential factors you have to keep in mind, then only you will be able to submit an online form 10D.
Some important factors are:
- Your UAN must be linked with your Aadhar and Bank KYC links.
- UAN number must be activated.
- Update profile photo and make sure that registered mobile number is active.
- E-nomination should be filed and verified using e-signature.
- Make sure to update the date of joining and leaving the job.
- Make sure that you have served for 10 years minimum.
Documents Required
- Before you apply for PF Form 10D, make sure to have the following documents attested with your downloaded Form 10D.
- Three photographs of the account holder (passport size).
- A detailed role of a pensioner along with the account holder’s signature/thumb impression. Make sure that the form is enclosed with the Claim form.
- You must submit the certificate and pay of the EPF member at the time they leave the job or get retired or unfortunately not alive anymore.
- And, if the employee has health issues permanent or complete disablement, they need to go under medical examination before the medical board assigned by the EPFO. And the appropriate documents must be attested with the form.
- And, if the company is closed and there is no officer appointed then the application will be forwarded to the magistrate, gazetted officer, bank manager or other authorized officer as approved or appointed by the Commissioner.
